SUMMARY
The Dental Assistant assures high-quality customer/patient service, quality dental care, clinical/administrative systems support, and positive, efficient work environment in the PCCS dental clinics. Position is responsible for assisting the dentist(s), dental hygienist(s) and dental therapist(s) in the direct provision of primary care dental services to patients of the center. The Dental Assistant is also responsible for sterilization, preparation and inventory control of dental instruments and supplies as well as keeping work areas clean.
PRIMARY ACCOUNTABILITIES
· Improve patient outcomes, and deliver quality care within practice areas as assigned. Ensure all services are delivered in a manner that meets or exceeds goals and expectations for clinical outcomes, quality assurance standards, and patient satisfaction.
· Ensures timely and accurate collection of patient data, and makes observations on assigned patients to provide supportive data in the patient assessment.
· Ensure all patient charts and related documentation are maintained current and relevant at all time.
· Ensure all lab practices are conducted consistent within current, relevant standards, as well as state and local laws and regulations, patient centered medial home model and related professional standards.
· Demonstrate competency in practice and knowledge of current standards of practice. Maintains credentials and performs current lab practice expectations within established guidelines.
· Provides care in a manner that respects the patient’s rights and choices in a multi-cultural setting.
· Develops and maintains favorable internal relationships, partnerships with co-workers, including dental hygienist and assistant staff, dentists, and support staff and business office staff.
· Develops and maintains favorable external relationships with vendors and contractors.
· Ensure all actions, job performance, personal conduct and communications model the highest standards of positive professionalism through unconditional positive regard for CRPC patients, clients, and personnel.
· Uphold and ensure compliance and attention to all corporate policies and procedures as well as the overall mission and values of the organization.
· Serves as dentists, dental hygienists and dental therapist’s chair-side assistant.
· Ability to make future patient appointments or referrals
· Prepares operatory for patient treatment as the dentist’s or dental therapist’s directions.
· Abide by policy and procedure of the PCCS in-conjunction with OSHA regulations
· Digital Radiography in accordance with state regulations and law as well as Dental Clinic protocol.
· Performs independent procedures as delegated and directed by the dentist.
· Maintains dental equipment in accordance with manufacturer’s directions and Dental Clinic policy.
· Maintains adequate operatory supplies and compiles a list of individual item shortages for inventory control and ordering purposes.
· Maintains a list of all Dental Department patients, monitors patient flow, and assists the dentists, dental therapists, and dental hygienists in assuring that all patient records and documents are properly and accurately completed and filed.
· Receives and places necessary telephone calls consistent with professional matters, clinic business and patient care.
· Assists with various clinical and administrative functions of the clinic as appropriate and time permits.
· Insures the sterility of all reusable dental instruments and equipment in accordance with clinic protocol.
· Insures the proper disposal of all contaminated or potentially contaminated materials in accordance with Clinic policy as well as state and federal regulation(s).
Knowledge, Skills and Abilities
Comprehensive knowledge of dental assisting techniques and procedures for general dentistry.
Knowledge of modern dental materials, their storage, handling and applications is required.
Cognizant of the expectations and concerns of the clinic’s dental patients and able to respond in an empathetic and professional manner.
The ability to work with persons from a wide diversity of social, ethnic and economic backgrounds is necessary.
Ability to attend to multiple tasks at the same time and to prioritize assignments and responsibilities to ensure compliance with established deadlines and protocols.
Effective oral and written communication skills in English are required.

JOB SUMMARY
The Popup Provider will be the lead clinician at NHS popup clinical events. These are events that are led by NHS staff at locations around the Twin Cities, mostly non-clinical, such as schools or community centers. Most of the clinical care will be for children—assessing and recommending vaccines and doing well-child checks. However, we may add other services for adults or children, in the future. NHS will work with the Popup Provider to set a calendar of quarterly events, based on both clinic and provider availability.
Due to the nature of community-based popup clinics, the Popup Provider will be asked to demonstrate flexibility and clear decision-making, leading the NHS team in delivering quality health care. The Popup Provider will refer patients and follow up to NHS clinical providers as needed. They will need to complete training and credentialing with NHS staff prior to providing care.
NHS’ electronic medical record is Athena. If the Popup Provider is not experienced in Athena, NHS will provide a scribe to support the clinical care.
ESSENTIAL FUNCTIONS:
Provides comprehensive health care for clinic patients as established by clinic protocols and applicable community practice standards. These services include health maintenance; acute episodic care; long-term care of stable chronic illness; ordering appropriate lab tests, x-rays and referrals; diagnosis, planning and evaluation as appropriate.
Serves as the sole provider at NHS popup clinic events, which will primarily take place on evenings and weekends
Consults with or refers to Medical Director, other clinic physicians, other nurse practitioners, specialists, certified nurse midwives, and/or other health care personnel as patients' needs may warrant
Secures patient health and developmental histories
Performs appropriate physical examinations
Maintains patient records as established by clinic protocol

Hennepin County is seeking a NorthPoint Health & Wellness Chief Executive Officer to lead, develop and sustain the community health and human services transformational change efforts in North Minneapolis through the operation of its Federally Qualified Health Center (FQHC).
For more than 50 years, NorthPoint Health & Wellness Center in North Minneapolis has made history as it partners with community to improve health outcomes. NorthPoint centers the well-being of its largely Black, Indigenous, Latino/Latinx, Asian, Immigrant and LGBTQIA+ patient and participant population by offering medical, dental, behavioral health and human services.
The position functions under the general direction of the NorthPoint Health & Wellness Center Community Board pursuant to the Co-Applicant agreement between the Community Board and Hennepin County related to the operation of the Federally Qualified Health Center (FQHC), directing the programmatic services delivered through NorthPoint Health & Wellness Center Inc., and establishing effective community programming and public relations efforts on behalf of the Community Board.
This position is also accountable to the Hennepin County Board of Commissioners and is under the direction of the County Administrator related to the ongoing financial and human resources functions of the FQHC operated within the Hennepin County Health line of business.

The candidate for this position is responsible for the following:
Planning, organizing, and managing all aspects of revenue cycle to ensure timely, compliant, and accurate billing while optimizing activities directly impacting revenue cycle performance.
Providing strategic and tactical planning, analysis, continuous quality and productivity improvements in order to meet key performance indicators established for the organization, and effectively achieve performance goals through collaborating with internal and external stakeholders.

Essential Functions and Responsibilities:
Provides support for core revenue cycle process (30%)
Organize and direct all revenue cycle operational activities to ensure control and management of revenue cycle, which includes charge capture, medical coding and coding education, claims production and clearing house, billing, collections, customer service, denial management, cash posting, follow-up, self-pay bad debt placement, Accounts Receivable (AR) management, and patient financial services.
Work closely with Revenue Cycle Analyst to optimize revenue cycle workflows and manage ongoing changes within the OCHIN Epic system related to revenue cycle.
Ensure patient complaints are addressed timely and debriefed to the department as part of ongoing initiatives to provide for high patient satisfaction and the voice of the patient as part of ongoing department communications.
Develop and maintain patient financial services for the organization that will optimize program enrollment with CUHCC’s sliding fee discount program and Minnesota Health Care Programs.
Develop and implement outreach programs to patients who are uninsured to educate and support them through the application process.
Maintain appropriate internal control safeguards related to revenue cycle including, accounts receivable, collection of cash, charge capture, coding compliance to quality standards.
Ensure compliance with relevant regulations, standards, and directives from regulatory agencies and third-party payers and support the Director of Compliance with any compliance reviews or inquiries.
Maintain current policies and procedures for all departmental functions and implement new procedures as needed. Ensure compliance with the Health Resources and Services Administration (HRSA) protocols are met at all times.
Ensures that policies, procedures, and controls are adhered to in accordance with CUHCC and University of Minnesota standard practices.
Team and Organizational Leadership (30%)
Manages billing, coding and patient financial advocate teams and monitors performance related to department productivity and quality as well as creating efficiency leveraging technology to achieve leading practice performance.

Lead the Revenue Cycle Steering Committee with preparation and facilitation of committee meetings and provide support to Committee stakeholders to meet target expectations adopted by the Committee.
Provides subject matter expertise related to revenue cycle operations to the organization and represents CUHCC with external stakeholders.
Provide for revenue cycle representation on various committees within the organization and support the need for internal and external reporting related data provided within the revenue cycle operations (i.e., Medicare Cost Report, Uniform Data System (UDS) reporting, third-party payer, and contract audits, etc.).
Reporting, Analytics and Performance Measures (30%)
Establish performance metrics and Key Performance Indicators (KPIs) that align with benchmark standards and result in daily maximum output supporting a strong revenue cycle for the organization. Ensures the organization monitors and measures performance by providing timely and relevant data that result in improving outcomes for the organization.
Oversees functions, priorities, and staff effectiveness to ensure maximum efficiency and outcomes are achieved and work is executed in a compliant, accurate and timely manner.
Perform daily analysis of organizational status related to Pre-AR, accounts receivable including third party payment performance and the timeliness of cash collections and posting.
Support the month end close process to ensure all patient encounters are accurately reported within the period of performance.

Dentists provide general and emergency pediatric and adult dentistry care to CUHCC’s (Community-University Health Care Center) diverse patients and serve as a preceptor to dental students and residents from the University of Minnesota. Services include a broad range of diagnostic, restorative, and oral surgery to pediatric and adult patients. Each dentist is expected to provide exams, training, and/or oversight to a hygienist and student or therapist as well as a dental assistant(s). In addition, CUHCC dentists must sign the Collaborative Management Agreement for CUHCC’s Advanced Dental Therapist(s). As clinical providers, dentists participate on quality improvement projects and other committees as designated. CUHCC dentists must provide respectful care to diverse populations and be able to meet the productivity and quality measures established by the clinic. This will require each dentist deliver care to multiple chairs simultaneously and collaborate with other members of the oral health care team.

Maintain accurate and timely notes in the electronic health record.

As part of its integrated care model, Community-University Health Care Center (CUHCC) has formed a Community Health Worker Team that helps individuals to achieve their health goals and supports population health at CUHCC and and in the community. CHWs are fully integrated members of patients’ care health teams. Using their cultural knowledge, language skills, life experiences and training, CHWs support patients with health education, skill-building and health care navigation through individual visits and group classes/meetings. CHW will also conduct outreach activities to promote health, assist with community-based care and/or serve as a bridge back to the clinic. CHW work occurs primarily during regular workday hours, but some evening and weekend hours may be available or required. CHW will conduct their work in the clinic, and patients’ homes, and community settings (e.g., schools, public housing, food shelves). There may be some flexibility for working remotely.

ROLE:
This individual serves as the central facilitator to patients assigned to their care. Their general function is to guide and coordinate care and collaborate with primary care providers, RNs, MAs, and clinic staff in identifying needs, organizing activities, and organizing plans of care for patients with behavioral health conditions. They are an advocate for the patient and work to link them with health and community resources that provide a range of services, promote self-management, improve health, and reduce disparity. They assist patients to achieve health goals and improve health care outcomes.
JOB DUTIES:
Serve as a key point of contact for discussing and enrolling patients into the SCHS behavioral health program (document the patient’s acceptance or denial).
Work directly with patients and their primary care provider(s) and other clinic team members to build and maintain the patient care plans.
Manage depression/anxiety measures and follow ups (in clinic or by phone) in accordance with Minnesota quality standards.
Build relationships with patients, patient’s families, guardians, etc.
Introduce patients to the wide variety of services available across the organization as well as
identifying outside resources and services as necessary.
Serve as an advocate for the patient in understanding needs surrounding shelter, child care,
transportation, safety and other social determinants of health.
Conduct basic screenings of patients within primary care, including review of barriers to health self-management, depression, stress and substance use.
Provide health education and coaching to patients and refer patients to other care team members (nurses, patient advocates, primary care provider, outside specialists, etc.) as necessary. Topics discussed may include but are not limited to:
o Medication knowledge, adherence and accessibility
o Benefits of best practice treatment for their chronic conditions
o Encourage the patient to identify and set specific health goals in conjunction with goals set by their care provider(s).
Carry out structured clinical protocols with patients in person or by telephone.
Intervene appropriately in crisis prevention and de-escalation.
Serve as a bridge between the patient and internal and external referrals. May work directly with the referral specialist to connect the patient to external community agencies and referral sources when indicated by the care team.
Maintain and document regular communications with patients in the electronic health record.
Manage and work inpatient and emergency room tracking and facilitate appropriate follow ups as needed.
Manage tracking spreadsheet for reporting and follow up.
Contact enrolled patients on a monthly basis. Make additional inquiries/contacts as needed or
requested by the patient and/or their care team.
Work collaboratively with clinic staff to implement and refine workflows throughout the clinic

Continuously seek, build and maintain relationships with community partners and resources.
Determine if patient has existing insurance coverage; discuss payment options and schedule appointment with patient advocate, if necessary.
Manage wait list on Epic and follow up.
Ensure patient exam rooms are fully restocked with BH business cards, DV resources, child development resources, post-partum education and self-harm resources.
Prepare for next day visits through pre-visit planning process to verify if service plans or forms such as PHQ, SDQ, CASII and ECSII need to be updated.
Enter completed paper work into MN-ITS and ensure paper work is scanned into patients chart.
Create binders with DV resources for patients and other clinic team members update and refill them when necessary.
Assist with other duties and responsibilities as assigned.

Location and hours:
