Description
Hennepin County Health Care for the Homeless is seeking a Street Outreach Nurse who will be responsible for leading panel management and care coordination for patients of the Health Care for the Homeless (HCH) Medical Street Outreach program. We are looking for someone who is skilled and experienced in working with individuals experiencing homelessness in unsheltered settings, to support the team’s response to the public health priorities of the opioid crisis and infectious diseases related to drug use (e.g. the current HIV outbreak among injection drug users and people experiencing unsheltered homelessness). In this role, the Street Outreach Nurse will meet with patients where they are: on the street and/or in encampments in the Minneapolis area to provide care with the goal of improving health outcomes for individuals affected by these public health priorities. Working closely with medical providers, this position will assess patient’s health care needs and provide care such as medication management, point of care labs, wound care, and vaccinations as well as connect patients to health care homes for ongoing care.

In this position, you will:
Work collaboratively with the HCH Medical Outreach Team in following the objectives as related to the public health priorities of the opioid crisis and drug related infectious disease work and do the following:
Engage with patients who are unsheltered or are adjacent to the unsheltered community who do not seek traditional methods of health care.
While building relationships and trust, promote harm reduction while identifying health needs, and assess readiness to engage in health care.
Offer focused street medicine through a trauma-informed and harm-reduction lens, which may also include the facilitation of access to social services including needs assessment, housing, case management, benefits, legal help, and assistance with basic needs.
Partner with HCH medical providers to help patients access medications specific to the public health priorities, primarily: medications for opiate use disorder, antiretroviral medications for patients living with HIV, pre-exposure prophylaxis for HIV, screening/treatment for sexually transmitted infections, and treatment of other infections related to drug use.
Collaborate with other community street outreach agencies and Hennepin County departments and participate with staff on broader county projects.

SUMMARY:

JOB SUMMARY:
The Pharmacy Technician supports the Pharmacy department by assisting pharmacists in their daily tasks. •Schedule clinic visits by the procedure according to the established schedule for each provider. Accurately register each patient in the EMR for the clinic visit, using a thorough interview technique. Assure all patient demographic and insurance information is current and complete for billing. Obtain patient consent for treatment. Ensure all compliance requirements are met. Assist patient with EMR when applicable. Obtain patient signature on any ABN’s by SCHS’s prior authorization dept.
•Maintain full understanding and follow SCHS and applicable HIPAA regulations. Maintain constant awareness of patient confidentiality in all scenarios. Uphold and conform to all regulations without exception.
•Complete assigned daily duties in a timely manner.
•At least 24 hours prior to patient appointment, ensure the pre-visit process is followed.
•Greet patients and update registration information at the time of visit; assist new patients when necessary in completing registration forms.
•Perform cash collections at the time of service, consistent with financial counseling procedures, SCHS’s credit and collection policy, and any other financial directives. Utilize software for payment and receipt. Handle money competently and securely.
•Assist patients with billing questions to the best of your knowledge and direct any additional questions to the billing department.
•Assign established charges to provider activities; total account balances and collect payments for services rendered at time of visit. (applicable at dental clinic)
•Verify insurance information prior to the visit and upon registration/check-in; identify opportunities when a patient may benefit from meeting with enabling services (Patient Advocate); dental clinic PCR’s must also assure charge sheet information is complete before turning over to the billing clerk.
•Serve and participate as requested in staff meetings, continuous quality improvement, and other process development opportunities within the organization.
•Ensure that the front office area is kept clean and orderly.

This position will work closely with our mental health and primary care providers and together create a true healthcare home. Onsite therapy and telehealth therapy will be provided to patients. SUMMARY:
Under the supervision of the Dentist, performs clinical dental hygiene and related services, off-site outreach programs and other related work as assigned by the Dentist and/or Chief Operating Officer (COO).
MINIMUM QUALIFICATIONS:
Completion of an accredited dental hygiene course, RDH license, and Dental Hygiene licensure in Minnesota.
KNOWLEDGE, SKILLS, AND ABILITIES:
Good clinical skills in adult and child prophylaxis, management of Class I and II periodontal patients and diagnostic x-rays.
Ability to recognize pathology and perform charting and preliminary exams. Skill in patient dental health education and oral hygiene instruction. Ability to establish and manage efficient scheduling.
Ability to effectively communicate with people of cultural and socioeconomic diversity.
Ability to interact positively with staff. Ability to keep current with developments in dentistry.
Bilingual preferred
RESPONSIBILITIES:
Performs dental cleanings including scaling, polishing, and root planning.
Screens patients for medical conditions, oral pathology and periodontal disease.
Exposes clinically diagnostic x-rays.
Gives oral hygiene instructions to patients.
Completes relevant forms in dental charts.
Polishes restorations, places temporary restorations and dental sealants.
Follows sterilization protocols.
Maintains performance standards and assists in quality assurance.
Performs dental outreach activities.

ROLE: The Behavioral Health Consultant (BHC) provides on-site therapeutic services to all ages. The BHC will also act as Behavioral Health Therapist for a few hours each week.
JOB DUTIES:
Work closely with all members of the healthcare team in development of a plan of treatment.
Provide short term, targeted intervention to medical patients.
Facilitate individual, couples and family sessions in outpatient setting.
Develop treatment plan for patients.
Develop and maintain thorough knowledge of referral resources throughout the service area.
Maintain accurate and timely records, including clinical progress notes, according to clinic policy.
Provide positive customer relations in dealing with patients, families, physicians, members of the healthcare team, referral sources, service agencies and others.
Attend and participate in monthly clinical consultation meetings.
Attend and participate in staff meetings and committees.
Consult with Behavioral Health Director when necessary and appropriate.
Assist with other duties and responsibilities as assigned.

Pulp vitality testing of teeth requiring further diagnosis
The application of desensitizing medication or resin
Fabrication of athletic mouth-guards
Placement of temporary restorations

Tissue conditioning and soft reline as required by a patient.
Atraumatic restorative therapy

Dental Therapist Indirect Supervision:

Preparation, placement and cementation of preformed crowns

Oral Evaluation and assessment of dental disease and the formulation of an individualized treatment plan; as authorized by the collaborating dentist.

We are seeking a Family Dentist at our locations in Bigfork and Cook to provide quality care in our communities. This Dentist will become part of a general dental practice, providing care in all areas of dentistry for all ages according to their comfort level and expertise. Collaborate with excellent dental specialists in the region for patients who need to be referred for care. Utilize our electronic dental records software that is fully integrated with our electronic medical records, improving patient care and communication between our medical and dental providers. All necessary training in electronic medical records will be personalized to fit individual needs. In modern facilities, you will enjoy a patient focused practice with a collaborative team of Dentists. Clinic schedules and patient appointments are quality focused creating a rewarding practice.
Scenic Rivers Health Services has 4 dental clinics, 7 outpatient medical clinics including Physicians who staff two Critical Access hospitals with ER Coverage; and behavioral health counselling. By offering a diverse set of services we have been able to create financial stability for our organization. Further, we have been able to procure the proper PPE and make the appropriate practice adjustments in our Dental clinics during this challenging Coronavirus environment. We are working to resume elective dental care by mid-June and are currently offering Emergency, followed by Essential dental care.
Schedule:
4.5 day work week (full time); reduced schedule for part time
Approximately 60 Minute Fillings, Extractions, Toothaches, Denture visits and Impressions and Crown Preps
Appointment lengths can be adjusted to provider’s needs and complexity.
Average Volumes are 8-10 patients per day
Requirements:
Doctor of Dental Surgery (DDS) degree or Doctor of Dental Medicine (DMD) degree
Current or eligible for MN License, DEA certificate
Current or able to acquire Nitrous Certification
At least one year of experience as a Dentist preferred
