The Popup Provider will be the lead clinician at NHS popup clinical events. These are events that are led by NHS staff at locations around the Twin Cities, mostly non-clinical, such as schools or community centers. Most of the clinical care will be for children—assessing and recommending vaccines and doing well-child checks. However, we may add other services for adults or children, in the future. NHS will work with the Popup Provider to set a calendar of quarterly events, based on both clinic and provider availability.
Due to the nature of community-based popup clinics, the Popup Provider will be asked to demonstrate flexibility and clear decision-making, leading the NHS team in delivering quality health care. The Popup Provider will refer patients and follow up to NHS clinical providers as needed. They will need to complete training and credentialing with NHS staff prior to providing care.
NHS’ electronic medical record is Athena. If the Popup Provider is not experienced in Athena, NHS will provide a scribe to support the clinical care.
ESSENTIAL FUNCTIONS:
Provides comprehensive health care for clinic patients as established by clinic protocols and applicable community practice standards. These services include health maintenance; acute episodic care; long-term care of stable chronic illness; ordering appropriate lab tests, x-rays and referrals; diagnosis, planning and evaluation as appropriate.

For more than 50 years, NorthPoint Health & Wellness Center in North Minneapolis has made history as it partners with community to improve health outcomes. NorthPoint centers the well-being of its largely Black, Indigenous, Latino/Latinx, Asian, Immigrant and LGBTQIA+ patient and participant population by offering medical, dental, behavioral health and human services.

The candidate for this position is responsible for the following:
Planning, organizing, and managing all aspects of revenue cycle to ensure timely, compliant, and accurate billing while optimizing activities directly impacting revenue cycle performance.
Providing strategic and tactical planning, analysis, continuous quality and productivity improvements in order to meet key performance indicators established for the organization, and effectively achieve performance goals through collaborating with internal and external stakeholders.
Leading and mentoring staff to achieve and maintain revenue cycle targets based on industry benchmarks.

Essential Functions and Responsibilities:
Provides support for core revenue cycle process (30%)
Organize and direct all revenue cycle operational activities to ensure control and management of revenue cycle, which includes charge capture, medical coding and coding education, claims production and clearing house, billing, collections, customer service, denial management, cash posting, follow-up, self-pay bad debt placement, Accounts Receivable (AR) management, and patient financial services.
Work closely with Revenue Cycle Analyst to optimize revenue cycle workflows and manage ongoing changes within the OCHIN Epic system related to revenue cycle.
Ensure patient complaints are addressed timely and debriefed to the department as part of ongoing initiatives to provide for high patient satisfaction and the voice of the patient as part of ongoing department communications.
Develop and maintain patient financial services for the organization that will optimize program enrollment with CUHCC’s sliding fee discount program and Minnesota Health Care Programs.
Develop and implement outreach programs to patients who are uninsured to educate and support them through the application process.
Maintain appropriate internal control safeguards related to revenue cycle including, accounts receivable, collection of cash, charge capture, coding compliance to quality standards.
Ensure compliance with relevant regulations, standards, and directives from regulatory agencies and third-party payers and support the Director of Compliance with any compliance reviews or inquiries.
Maintain current policies and procedures for all departmental functions and implement new procedures as needed. Ensure compliance with the Health Resources and Services Administration (HRSA) protocols are met at all times.

Objective/Overview
Dentists provide general and emergency pediatric and adult dentistry care to CUHCC’s (Community-University Health Care Center) diverse patients and serve as a preceptor to dental students and residents from the University of Minnesota. Services include a broad range of diagnostic, restorative, and oral surgery to pediatric and adult patients. Each dentist is expected to provide exams, training, and/or oversight to a hygienist and student or therapist as well as a dental assistant(s). In addition, CUHCC dentists must sign the Collaborative Management Agreement for CUHCC’s Advanced Dental Therapist(s). As clinical providers, dentists participate on quality improvement projects and other committees as designated. CUHCC dentists must provide respectful care to diverse populations and be able to meet the productivity and quality measures established by the clinic. This will require each dentist deliver care to multiple chairs simultaneously and collaborate with other members of the oral health care team.

As part of its integrated care model, Community-University Health Care Center (CUHCC) has formed a Community Health Worker Team that helps individuals to achieve their health goals and supports population health at CUHCC and and in the community. CHWs are fully integrated members of patients’ care health teams. Using their cultural knowledge, language skills, life experiences and training, CHWs support patients with health education, skill-building and health care navigation through individual visits and group classes/meetings. Hennepin County Health Care for the Homeless is seeking a Street Outreach Nurse who will be responsible for leading panel management and care coordination for patients of the Health Care for the Homeless (HCH) Medical Street Outreach program. We are looking for someone who is skilled and experienced in working with individuals experiencing homelessness in unsheltered settings, to support the team’s response to the public health priorities of the opioid crisis and infectious diseases related to drug use (e.g. the current HIV outbreak among injection drug users and people experiencing unsheltered homelessness). In this position, you will:
Work collaboratively with the HCH Medical Outreach Team in following the objectives as related to the public health priorities of the opioid crisis and drug related infectious disease work and do the following:

JOB SUMMARY:
The Pharmacy Technician supports the Pharmacy department by assisting pharmacists in their daily tasks. This position involves receiving prescriptions, both over the phone and in-person, entering prescriptions into a computer database, dispensing medications, assisting clients at the cash register, working with insurance companies and maintaining inventory. The Pharmacy Technician also ensures efficient workflows and complies with all security measures and quality standards.
Position Responsibilities:
Assists the pharmacist in providing optimal pharmaceutical care to the patient.
Accurately and efficiently processes prescriptions.
Interacts with patients and managed care plans to authorize prescription coverage.
Assembles prescriptions and follows proper procedures to prepare and dispense medications.
Accurately and efficiently completes patient purchase transactions utilizing point of sale systems.
Assists with maintaining accurate medication inventories and billing of medications.
Effectively communicates utilizing established systems and processes.
Performs data entry, adjudication, filling, labeling and bagging of prescriptions.
Processes refills requests and prior authorizations.
Performs daily ordering and inventory management.
Ensures pharmacy environment is clean and free of clutter, safe, and presents a professional atmosphere. Reports discrepancies or inaccuracies to pharmacist or supervisor.
Completes audits for days’ supply, signatures, dates, take as directed and returns-to-stock.
Documents and complies with standards for any active accreditations.
Answers phone calls, assist patients in answering their questions and directs calls to pharmacist if needed.
Maintains confidentiality of all employee, patient, and other privileged information regarding UFPHC business activities and according to HIPAA standards.
Performs other related duties as assigned.

Active technician registration with the Minnesota Board of Pharmacy.
PHYSICAL DEMANDS:
Ability to perform light lifting (up to 15 pounds) and repetitious physical activity.
Ability to stand for long periods of time.
