JOB SUMMARY:
We are seeking a dedicated Pharmacist to join our healthcare team at United Family Medicine for two months. As a Pharmacist, you will play a critical role in ensuring the safe and effective use of medications for our patients. Your responsibilities include dispensing medications, reviewing prescriptions for accuracy, providing patient counseling, oversight of one Pharmacy Technician, and collaborating with other healthcare professionals to deliver comprehensive care. To excel in this role, you should have a PharmD degree, a current state pharmacy license, and a strong knowledge of pharmaceuticals and medication management. ESSENTIAL FUNCTIONS
Processing incoming prescriptions by reviewing them for accuracy and updating patient records.
Dispensing medications to patients in accordance with physician orders, including creating customized medication administration regimens based on patient needs.
Educating patients about their medications and treatment plans, including how to take medications properly and how to recognize side effects.
Explaining new medications or new uses for existing medications to patients, physicians, or other health care providers as needed
Controls medications by monitoring drug therapies; advising interventions.
Completes pharmacy operational requirements by organizing and directing technicians’ workflow, verifying their preparation and labeling of pharmaceuticals, and verifying order entries, charges, and inspections.
Provides pharmacological information by answering questions and requests of health care professionals and counseling patients on drug therapies.
Complies with state and federal drug laws as regulated by the state board of pharmacy, the drug enforcement administration, and the food and drug administration by monitoring nursing unit inspection.
Maintains records for controlled substances and removes outdated and damaged drugs from the pharmacy inventory.
Supervises the work results of support personnel.
Maintains current registration, studies existing and new legislation, anticipates legislation, and advises management on needed actions.
Protects patients and technicians by adhering to infection-control protocols.
Maintains safe and clean working environment by complying with procedures, rules, and regulations.
Maintaining inventory of prescribed medications and other medical supplies and materials used in the practice of pharmacy.
Contributes to team effort by accomplishing related results as needed.
Performing other duties related to the operation of a pharmacy.

SUPERVISORY RESPONSIBILITIES:
Pharmacy staff.
KNOWLEDGE, SKILLS AND ABILITIES:
Demonstrated ability to effectively communicate with patients, all levels of personnel, and the public, orally and in writing, including interpretation of drug information materials and policies and procedures and production of written routine and ad hoc reports and correspondence.
Excellent organizational and time management abilities.
Demonstrated competency, education, training and experience in the provision of high-quality pharmacy services to a wide variety of patients throughout all lifecycles, through dispensing and clinical interpretations and interventions.
Effective interpersonal skills, including the ability to affect collaborative alliances, promote teamwork and ensure a high degree of internal and external customer satisfaction.
Knowledge and understanding of the Minnesota State Board of Pharmacy prescription regulations.
Ability to work as a team player or autonomously with strong organizational skills, project management skills and attention to detail.
Ability to work with vendors and other departments to keep multiple tasks on track and bring projects successfully to completion.
Strong skills in standard Microsoft Office applications (Excel, Word, Outlook)
Ability to initiate and follow through on complex programs of both short- and long-term duration.

Ability to perform the job in accordance with United Family Medicine’s Standards of Business Conduct, which includes compliance, ethics and integrity, confidentiality, protection of assets and avoidance of conflicts of interest and inappropriate business relationships.
EDUCATION/EXPERIENCE:
Doctoral Degree in Pharmacy (Pharm D) is required.
At least one year working as a pharmacist in a clinical pharmacy setting is required.
Focus on direct patient care, medication management and consulting is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS:
Current and unrestricted Minnesota Pharmacy license is required.
BLS certification is required.
APhA Immunization Certification is required.
Board Certification in BPS recognized specialty is preferred
Certificate in Physical Examination is preferred
Contraceptive Prescribing Certification is preferred
Maintain all criteria of DSCHC and contracted health plan credentialing process.
PHYSICAL DEMANDS:
Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift up to 15 pounds at times.
WORK ENVIRONMENT:
Work is performed in a clinic office environment. Contact with staff, patients and outside agencies. Possible exposure to communicable disease and medical preparations common to clinic environment.

JOB SUMMARY:
The Pharmacy Technician supports the Pharmacy department by assisting pharmacists in their daily tasks. This position involves receiving prescriptions, both over the phone and in-person, entering prescriptions into a computer database, dispensing medications, assisting clients at the cash register, working with insurance companies and maintaining inventory. The Pharmacy Technician also ensures efficient workflows and complies with all security measures and quality standards.
Position Responsibilities:
Assists the pharmacist in providing optimal pharmaceutical care to the patient.
Accurately and efficiently processes prescriptions.
Interacts with patients and managed care plans to authorize prescription coverage.
Assembles prescriptions and follows proper procedures to prepare and dispense medications.
Accurately and efficiently completes patient purchase transactions utilizing point of sale systems.
Assists with maintaining accurate medication inventories and billing of medications.
Effectively communicates utilizing established systems and processes.
Performs data entry, adjudication, filling, labeling and bagging of prescriptions.
Processes refills requests and prior authorizations.
Performs daily ordering and inventory management.
Ensures pharmacy environment is clean and free of clutter, safe, and presents a professional atmosphere. Reports discrepancies or inaccuracies to pharmacist or supervisor.
Completes audits for days’ supply, signatures, dates, take as directed and returns-to-stock.
Documents and complies with standards for any active accreditations.
Answers phone calls, assist patients in answering their questions and directs calls to pharmacist if needed.
Maintains confidentiality of all employee, patient, and other privileged information regarding UFPHC business activities and according to HIPAA standards.
Performs other related duties as assigned.

Active technician registration with the Minnesota Board of Pharmacy.
