We are excited to be hosting the Gillette Children's Pediatric to Adult Health Care Learning Collaborative Summit, which is the culmination of the eight-month, virtual learning series.
This conference will be held Thursday, June 13, 8:15AM – 4:45PM, at the Holiday Inn & Suites in St. Cloud, Minnesota.

Analyze evidence-based resources and apply tangible skills in the implementation of health care transition processes within your own organization.
Employ trauma-informed care and health equity principles into health care transition processes.
Gain insights from other organizations, nationally and from Minnesota, who initiated implementation of the Six Core Elements of Healthcare Transition.
