New Heart Health Funding Opportunity Available

The Cardiovascular Health Unit at the Minnesota Department of Health is seeking applications for a grant that supports community-based solutions to address heart health among residents of St. Paul’s Rondo and Capitol Heights neighborhoods.This grant opportunity focuses on health disparities, social determinants of health, and implementation of key strategies and objectives outlined in Minnesota's Action Plan to Address Cardiovascular Disease, Stroke, and Diabetes 2035.

Grants will range from $25,000 - $50,000 annually from January 2025 through September 2027.It is focused on reaching communities who are disproportionately burdened by heart disease and its risk factors, including high blood pressure.Learn more: Innovating from our Hearts to a Healthier Rondo.The Request for Proposals emphasizes connecting clinical and community services through increasing referrals to culturally appropriate programs and resources.  Organizations must demonstrate experience addressing health equity with all work focused on the Rondo and Capitol Heights neighborhoods in St. Paul. Public health and community-based organizations are encouraged to apply.

Applications are due Oct. 30, 2024.
