JOB SUMMARY

This position exists to provide oversight of health program staff and activities to support IHB’s Health Care Home model of team-based care and coordinated service. The Health Program Manager will manage grants and staff that support this care model, while working with internal and external stakeholders to ensure that IHB is providing patient centered care and promoting health equity. This position will also be responsible for clinical quality improvement initiatives.

ESSENTIAL JOB FUNCTIONS

  1. Manage and coordinate the daily activities of Health Program staff including RNs, Certified Diabetes Educators, Dieticians, and/or Community Health Workers.
  2. Responsible for staff development, personnel management, scheduling, performance reviews, training, conducting monthly team meetings, and other tasks associated with managing personnel.
  3. Coordinate medical clinic quality improvement initiative, data, and reporting.
  4. Act as Project Manager for Health Care Home certification. Assist with coordination of staff and clinic procedures and workflow and update policies to meet Health Care Home standards. Facilitate communication with MDH as needed. Attend all necessary meetings, and submit all necessary documents required by MDH for recertification.
  5. Facilitate Health Program team meetings at least monthly.
  6. Attend department and IHB staff meetings and represent Health Program staff at monthly management meetings.
  7. Submit monthly department reports as required.
  8. Develop and assist in the implementation of grant related activities and reporting including the SDPI Diabetes Programs, diabetes prevention grant programs, and other health promotion related grant programs.
  9. Evaluate, organize, and order supplies and educational materials for the health programs.
  10. Maintain chart documentation in electronic health record.
  11. Act as Program Coordinator for ADA’s Education Recognition Program. Ensure the Diabetes Program’s Diabetes Self-Management Education program meets and maintains nationally recognized quality standards, completing all necessary reports and leading the reapplication as required.
  12. Responsible for fiscal management of Health Program budgets
  13. Provide individual health information, education, and support in-clinic to patients with chronic health conditions including diabetes.
  14. Assist in meeting regulatory standards regarding nutrition and health education of such organizations as the BPHC, Joint Commission and Indian Health Service.
  15. Participate in Performance Improvement activities as needed.

KNOWLEDGE, SKILLS, AND ABILITIES

  1. Manages time effectively and prioritizes task completion to meet deadlines
  2. Strong knowledge of pertinent governmental and funding regulations
  3. Ability to work positively, flexibly and cooperatively in a team effort to accomplish the goals of the Indian Health Board of Minneapolis.
  4. Excellent written and oral communication skills.
  5. Knowledge of and respect for American Indian cultures and other cultures served by IHB.
  6. Proficiency with Microsoft Office applications, especially Access and Excel.
  7. Ability to interpret, adapt and apply guidelines and procedures.
  8. Self-motivated with effective critical thinking skills.

QUALIFICATIONS

  1. RN and/or Dietitian licensure in the state of MN.
  2. 2+ years supervisory and project management experience preferred.
  3. 2+ years experience providing diabetes and related disease health education, especially in an ambulatory setting is desired.
  4. Valid MN Driver’s license.

PHYSICAL DEMANDS

The work requires some physical exertion such as long periods of standing, walking, recurring bending, crouching, stooping, stretching, reaching, or similar activities; recurring lifting of children up and in excess of 50 pounds. The work may require specific, but common, physical characteristics and abilities such as above-average agility and dexterity. The work environment involves high risks with exposure to potentially dangerous situations requiring a range of safety and other precautions.
