Introduction

United Family Medicine is seeking a full-time Director of Nursing to serve as a clinical expert and resource for management and clinical staff in developing and accessing nursing standards of care for all nursing and clinical staff at United Family Medicine. The Director of Nursing will provide or coordinate the provision of disease-specific educational support to patients and assist with in-service training for staff. In addition, this position is accountable for the coordination of nursing care and patient/family education via in-person and telephone-based care. The Director of Nursing supports professional nursing practice across the continuum of care to meet the needs of the patient and family by utilizing internal and external resources. This position will ensure that all processes, systems, workflow and training are in place to allow nursing and clinical staff to work at the top of their license, and ensure all competencies are met.

ESSENTIAL FUNCTIONS:

  • Develop nursing & clinical protocolsIn collaboration with the Clinical Floor Supervisor and Vice President of Medical Services determine the scope of practice for all UFM nursing and clinical staff ensuring all perform at the top of their license, develop standard work for all nursing and clinical roles (RN, LPN, CMA)
  • Develop, assess and monitor clinical competencies: Research, create and write nursing workflows and competencies that facilitate exceptional patient care and efficient processes in the clinic. Review protocols for RN’s, LPN’s and MA’s and update as needed. Assess individual staff skills and create a staff competency adherence and monitoring process. Conduct regular chart audits for RN’s, LPN’s and CMA’s and provides feedback to individuals.
  • Develop and facilitate delivery of nursing education: Create and deliver targeted training and education to all nursing and clinical staff on workflows, competencies, standard work, safety, and quality standards. Conduct the nursing safety portion of new employee orientation. Ensure annual nursing and clinical staff required education, licenses and certifications are up to date. Prepare and deliver monthly in-services to nursing and clinical staff. Maintain external nursing and primary care networks and regularly share updates with provider, nursing and clinical staff.
  • Providing nursing and clinical guidance and oversight: Field nursing practice questions for RNs, LPNs and the Clinical Floor supervisor, and provide guidance, as appropriate, Serve as a liaison to clinic quality and process improvement projects. Educate staff on EMR updates that relate to nursing and clinical staff, manage emergency preparedness activities for UFM – ensure all UFM staff are trained in protocols.
  • Perform other related tasks as needed.

Additional Duties:

  • Obtain instructor certification for Basic Life Support through the American Heart Association. Conduct initial and renewal courses for staff (approximately 70 people) on a bi-annual basis.
  • Implement from scratch evidence-based skills competencies for nursing and medical assistant staff to provide safe, efficient, and competent care to UFM patients including rooming, personal protective equipment (PPE) donning/doffing, hand hygiene, safe injection practices, etc.
  • Develop and manage a comprehensive orientation program for clinical staff which includes:
  • – Create, manage, and grow an orientation program for nurses, medical assistants, and community health workers that incorporates orientation goals, schedules, preceptor support, and weekly 1:1 meetings.
  • – Create, manage, and grow an orientation program for providers (NP, PA, and MA) which includes identifying orientation goals, scheduling a well-rounded shadowing and interdisciplinary schedule, and assigning preceptors based on the skillset of new provider.
  • Manage the employee online learning program HealthStream and assign appropriate education for all levels of staff from clinical to administrative.
  • Research, cost-compare, and secure a contract with a new occupational health partner to improve the onboarding process for incoming employees as well as reduce costs to UFM by identifying best-practices for occupational health services.
  • – Review vaccination records and create a referral to our occupational health partner for appropriate occupational health onboarding employment needs.
  • Research, develop, and create a comprehensive educational material for UFM patients in collaboration with clinicians and staff to support UFM programs and services.
  • Maintain N95 respirator fit testing logs, inventory, and write a policy regarding requirements and frequency of medical clearance and fit testing.
  • Function as a safety expert including:
  • – Write an OSHA-compliant Exposure Control Plan to comply with OSHA standards.
  • – Update and educate staff on policies surrounding handling blood and body fluids and sharps including handling a blood and body fluid exposure.
  • – Chair the organizational Safety Committee with members from all departments.
  • – Create a desktop inventory of Safety Data Sheets on over 140 on-hand inventory supplies to comply with OSHA standards.
  • – Meet with all new employees to walk through blood and body fluid exposure protocols, donning/doffing PPE, and performing hand hygiene within the first week of hire.
  • – Manage the emergency response process within the clinic including sourcing appropriate emergency resources, training staff on handling emergencies, and ensuring daily checks of safety equipment are conducted along with monthly inventory of all emergency supplies.
  • – Review OSHA compliance guidelines and update staff annually.
  • Function as the organizational expert for the COVID pandemic including:
  • – Research and write policy for providers, nursing staff, and interns regarding best practices for testing and quarantine protocols based on MDH and CDC guidelines.
  • – Create educational materials /keep staff updated on best practices while at work, which may include information from the MDH.
  • – Source eye protection for staff to wear during all clinical visits.
  • – Organize and lead meetings with COVID sick-clinic staff to create policies to improve patient care flow.
  • – Act as project manager and leader in mass COVID vaccination clinics that incorporate best practices and efficiencies.
  • – Organize and manage community vaccination events including staff scheduling and education, securing and transporting appropriate supplies, and managing flow to vaccinate the UFM community.
  • – Manage inventory, reporting, mandatory meetings and education to maintain compliance of COVID vaccine management with current MDH guidelines.
  • Function as mentor and educator to two pre-medical school interns including practice guidance, skills development, and organizing growth opportunities with UFM providers.

SUPERVISORY RESPONSIBILITIES:

In addition to the duties described above, specific RN related responsibilities include:

  • Supervise team of Registered Nurses in daily work activities.
  • Create work schedules and manage time-off requests to ensure staffing is sufficient for clinical operations.
  • Training and onboarding of new nursing staff.
  • Coaching, counseling, mentoring, junior staff.
  • Identify problem areas and recommend improvements.
  • Oversee patient care on day-to-day basis. Fill in for other RN staff where needed.

Other RN Related Functions:

  • Ensure accurate and timely refill of medications based on triage or urgent needs. Process refill requests received electronically from pharmacies and submit Prior Authorization requests as needed. Route medications nursing is unable to refill per scope of practice to providers for review.
  • Assist providers with assessment and treatment of patients for acute and chronic needs including education, wound care, acute and chronic disease management, etc.
  • Work with an interdisciplinary care team with providers, medical assistants, community health workers, diabetic educators, etc., to coordinate and provide care of patients for acute and chronic needs.
  • Care coordination through internal and external resources including home health agencies insurance companies, pharmacies, clinics, and hospitals to coordinate patient care throughout the healthcare spectrum.
  • Collaborate with community health workers to manage complex patients enrolled in Healthcare Home Program.
  • Review patient communications via MyChart to ensure timely responses from appropriate healthcare team members.

KNOWLEDGE, SKILLS, AND ABILITIES:

  • Excellent clinical and interpersonal skills to provide medical guidance to nursing and clinical staff
  • The ability to use positive interpersonal and communication skills to create a positive experience for our patients, external customers, internal service partners and clinical staff.
  • Knowledge of clinical equipment used in a primary care clinic.
  • Ability to manage operational policies and procedures and keep within scope of practice
  • Ability to work collaboratively on clinical issues.
  • Ability to identify problems and bring these to resolution.
  • Sound judgment and decision-making abilities.
  • Ability to create nursing and MA competencies and assessment protocols.
  • Ability to establish and maintain effective communication.
  • Knowledge of clinical equipment and database tools used for patient care.
  • Ability to perform support functions as needed.
  • Ability to prepare and maintain detailed records, files, reports, and other correspondence.
  • Ability to demonstrate initiative and work in conjunction with appropriate site management team members.

EDUCATION/EXPERIENCE:

  • Master’s in nursing field is required
  • Minimum of five (5) years of clinical or related nursing experience required
  • Minimum two (2) years of clinical experience in a lead or supervisor role is required.
  • Ambulatory care experience preferred.
  • Comprehensive education and training as a nurse trainer and educator is strongly preferred.
  • Experience developing nursing protocols, competencies, and workflows in a primary care clinic is strongly preferred.
  • Experience working with diverse patient and employee populations is strongly preferred.
  • Experience facilitating teams and gathering and assessing input from a variety of stakeholders is strongly preferred.

CERTIFICATES, LICENSES, REGISTRATIONS:

  • Current MN RN License is required.
  • Current BLS certification is required.

PHYSICAL DEMANDS:

  • Prolonged periods of sitting at a desk and working on a computer.
  • Must be able to lift up to 15 pounds at times.
