General Job Statement:

The Health Care Home Case Management Assistant works with primary care providers, clinic staff and other collaborative teams to assist in the identification basic needs, organize activities, and collaborate with other health care team members to coordinate and implement plans of care for patients. Case management activities will include services for women that are pregnant and/or have chronic conditions including their families in the effort of linking them with health and community services/partners that will promote self-management, improve health outcomes, and reduce disparity.

Minimum Qualifications:

  • Current Minnesota Medical Assistant certification or current Minnesota Community Health Worker certification.
  • Certification CPR desired or the ability to obtain in the first year of employment.
