THIS POSITION IS ELIGIBLE FOR A SIGN ON BONUS OF $1500!! FIRST HALF WILL BE PAID OUT ON THE FIRST PAYCHECK FOLLOWING 6 MONTHS OF EMPLOYMENT AND THE FINAL HALF WILL BE PAID OUT AFTER ON THE FIRST PAYCHECK FOLLOWING ONE YEAR OF EMPLOYMENT!

Minnesota Community Care exists to strengthen our community through providing health care for all. The Triage RN position is a critical role that serves our mission as clinical contact for patients and caregivers calling our call center with specific symptoms or patient concerns. This position will perform professional nursing practice functions (assessment, planning, implementation and evaluation) to provide timely, efficient, and accurate triage response and documentation in accordance with organizational protocols.

Responsibilities

  • Conduct patient telephone triage through use of nursing assessment skills, clinical background/knowledge, and utilizing decision support tools such as ClearTriage (Schmitt-Thompson telephone triage protocols), Centricity CDSS prompts, and Standing Orders to systematically assess and address patient needs and to support making evidence-based point-of-care decisions
  • Determines acuity/appropriate disposition e.g. same day or next available appointment needed, code orange, urgent care, ER, etc
  • Schedule acute patient appointments per appointment guidelines
  • Identify actual or potential health, safety, and educational needs and provide patient education as needed
  • Develop a collaborative plan of care with the patient, their support systems and their providers. Confirm patient understanding and comprehension of plan of care. Evaluate patient comfort level and agreement with plan of care, skills, and coping ability
  • Demonstrate communication that reflects good counseling, listening, questioning, negotiating and customer service/relation skills. Identify barriers to communication e.g. cultural, language, and education level and adapt communication to accommodate these needs
  • Documents advice provided and patient/caller response in EMR
  • Processes urgent prescription refill requests if pharmacy tech is not available
  • Provides normal test results to patients who are not able to talk to their primary care team in a timely manner
  • Collaborate with other members of the care team as needed
  • Serves as a clinical resource for non-licensed connection center staff

Physical Demands
While performing the duties of this job, the employee is continuously required to perform computer-related work. The employee is occasionally required to stand and walk. On occasion the incumbent may be required to stoop, bend or reach above the shoulders. The noise level in the work environment is usually moderate.

Qualifications

Required Education and Experience

  • Graduation from an accredited nursing program
  • Current Minnesota unrestricted RN license
  • CPR certification
  • EMR experience
  • Bilingual English/Spanish Required
  • Proof of COVID Vaccination Required, Religious and Medical exemption will be considered

Preferred Education and Experience

  • BA/S in Nursing Degree
  • At least one year of nursing experience in the ambulatory care setting
  • One year of phone triage experience preferred

  • Skills in assessment of patient condition and taking vital signs
  • Ability to meet and maintain the necessary background checks as aligned with position functions 
  • Skills in preparing and maintaining records and reports 
  • Ability to effectively handle high-stress fast paced environment

  • Strong collaboration skills, with the ability to work effectively in a team-based setting
  • Ability to respond appropriately to unpredictable situations and diverse patient populations
  • Excellent written and verbal communication skills 
  • Ability to communicate in the English and Spanish language for effective written and verbal correspondence in order to complete job functions as mentioned above
