United Family Medicine is seeking a full-time Director of Nursing to serve as a clinical expert and resource for management and clinical staff in developing and accessing nursing standards of care for all nursing and clinical staff at United Family Medicine. The Director of Nursing will provide or coordinate the provision of disease-specific educational support to patients and assist with in-service training for staff. In addition, this position is accountable for the coordination of nursing care and patient/family education via in-person and telephone-based care. The Director of Nursing supports professional nursing practice across the continuum of care to meet the needs of the patient and family by utilizing internal and external resources. This position will ensure that all processes, systems, workflow and training are in place to allow nursing and clinical staff to work at the top of their license, and ensure all competencies are met.


The Associate Medical Director (.6 FTE Clinic responsibilities/.4 FTE Administrative responsibilities) leads, coordinates and provides clinical oversight to family medicine providers. This position serves as a member of the clinic leadership team and as a clinical provider. The Associate Medical Director is responsible for the day-to-day medical operations of the clinic and the supervision of both MD’s and advanced practitioners. The Associate Medical Director is in charge of implementing and maintaining the highest standards of professional medical care for the patients at UFM.


  • Lead providers and medical staff towards the support and achievement of the medical, financial, quality, and operational goals and objectives of UFM.
  • Provide medical leadership to quality initiatives, compliance, and reporting for the clinic, with the Vice President of Medical Services.
  • Conduct regular chart reviews with MD and advanced practitioner staff.
  • Participate with the Vice President of Medical Services and CEO, in the recruiting and onboarding of new medical providers.
  • Provide medical leadership to physicians and direct service providers with respect to services and medical care, working hours, leave time, their supervision of personnel and interaction with patients.
  • Monitor and direct the achievement of productivity standards and goals to achieve organizational sustainability and growth in keeping with national FQHC standards.
  • Address clinical issues regarding patient satisfaction; in collaboration with the operations team, assure that the electronic record processes are supportive to patient care. Monitor that imputed data is appropriate for timely and effective patient care through review and monitoring.
  • Prepare clinical reports and correspondence concerning patients under his/her care or projects under her/his responsibility to governing bodies, physicians, hospitals, and other individuals and facilities.
  • Oversee the scheduling of medical staff, medical/provider staff productivity targets; patient flow and medical laboratory issues to assure the national FQHC productivity and clinical targets are met.
  • Establish, implement, and monitor adherence to basic medical requirements for Federally Qualified Clinics, OSHA, Infection Control, Joint Commission and other governmental/non-governmental regulatory and accrediting agencies as needed by UFM.
  • Maintain a panel of patients as a clinical practitioner.
  • Perform other duties as assigned.
  • Ex-officio board member: present reports to the board as requested by CEO; lead Quality and Compliance Committee in partnership with Vice President of Medical Services.
  • Sets and casts the vision, goals and objectives for the medical department, together with the Vice President of Medical Services, and decide which new clinical services, partnerships and initiatives should be part of the UFM Care Model.
  • Together with the CEO, the Director of Primary Care Services and the Vice President of Medical Services:
  • – Is responsible for the center’s medical relationship with hospitals and other care partners.
  • – Develops performance improvement metrics for UFM at both the system and practitioner level.
  • – Acts as a champion for patient-centered care and ensures that providers have appropriate training and development to be successful in the UFM care model.
  • – Informs executive leadership of trends, problems and opportunities in quality, compliance, safety and care coordination.
  • – Develop policies and process to oversee the administration and delivery of the provider performance and peer reviews. Monitors clinical performance in the medical record and when needed, advises medical providers on factors that affect clinical decisions.
  • – Conduct regular chart reviews.

Development Plans:

  • Provide support and leadership of the integration/coordination of other services provided by UFM in regard to clinical care (i.e., Mental Health, Dental, 340b drug program, Medical Home project, Community Partners outreach etc. Collaborate with the Vice President of Medical Services, Director of Primary Care Services and CEO, to assure that all programs meet FQHC standards.
  • Provides medical leadership and guidance of strategic projects and initiatives in collaboration with the CEO and the Vice President of Medical Services.
  • Provides clinical oversight and problem resolution in collaboration with the CEO and the Vice President of Medical Services. This includes but is not limited to all provider performance reviews and performance improvement plans, provider privileging, credentialing, and enforcement of regulatory requirement issues.


The Associate Medical Director provides leadership and direction for patient care and general oversight of UFM employed and contracted providers and provision of medical services by UFM provider staff, including physicians, advanced practitioners and behavioral health practitioners, in collaboration with the Vice President of Medical Services.


  • Interface clinical and administrative decision-making processes to achieve UFM patient-centered goals and objectives, using advanced problem solving, negotiation and decision-making skills.
  • Ability to communicate effectively both orally and in writing in determining solutions to challenges and barriers.
  • Ability to establish rapport and gain the confidence of healthcare leaders inside and outside of the organization.
  • Ability to self-direct, and carry out programs and projects in a timely manner
  • Demonstrate highest-level medical, administrative, and ethical practices toward the achievement of UFM priorities as a Federally Qualified Health Care Center.
  • Ability to manage provider and administrative performance toward priority goals and objectives.
  • Lead provider performance by example toward productivity and program achievements.
  • Interface clinic and administrative activities toward patient-centered care and delivery.
  • Ability to perform the job in accordance with United Family Medicine’s Standards of Business Conduct, which include compliance, ethics and integrity, confidentiality, protection of assets and avoidance of conflicts of interest and inappropriate business relationships.


  • Nurse Practitioner (CRNP), Physician Assistant, or MD plus 5 years patient care experience following graduate degree and/or certification in area of specialty or medical school is preferred.
  • Patient care experience in an outpatient internal medicine or family medical clinic including pediatrics, obstetrical/gynecological, medical care and non-emergency care is preferred.
  • Supervisory experience as a nursing or medical team lead and/or director, with successful previous experience as a nursing or physician leader, having demonstrated the essential functions of the role is preferred.
  • Graduate of accredited nursing or medical school is required.
  • Successfully completed 5 years working in medical practice is preferred.


  • Board Certification in Family Medicine or Internal Medicine is required.
  • Current unrestricted license to practice Nursing or Medicine in Minnesota is required.
  • Current unrestricted DEA license is required.
