Advanced Practice Provider (NP/PA)

Provides primary health care to patients in Community Health Service Inc. practice sites
Performs outpatient procedures within the scope of primary care.
May assist the Medical Director in the revision and implementation of practice principles, clinical protocols, and health care plans as needed.
Consults with staff nurse practitioners and physician assistants as directed.
Follows all applicable national patient safety goals in the provision of patient care.
Serves as a role model for effective teamwork among staff and actively works to maintain positive morale and attitude. Leads the team in the team-based care model
Provides care in collaboration with the center’s team of physicians, nurse practitioners, behavioral health providers, nurses, and medical assistants.
Serves as a resource person for clinical staff regarding patient care issues.
Establishes and maintains rapport with community agencies.
Performs duties essential to the position and program.

PHYSICAL REQUIREMENTS: (Initial by duties you are able to perform)
In an eight hour workday, the employee must be able to: bend and crouch occasionally,
sit continuously and stand or walk frequently.
  • Employees must be able to lift up to 50 pounds occasionally.
  • Employees must be able to use hands/arms, feet/legs for repetitive fine to large motor skill movements.
NP/PA with 2-3 years experience in an outpatient clinical setting.
Valid state medical license/DEA in the jurisdiction of a clinic required.
Basic Life Support (BLS) is required.
Ability to diagnose and treat conditions within the scope of full-spectrum primary care.
Knowledge and understanding of the principles and standards of clinical practice in outpatient medicine.
Thorough knowledge of health promotion and disease prevention strategies.
Ability to work as a team member, establishing and maintaining effective relationships with patients and other staff.

Experience in working in community health centers, public health, or in a similar type of program.
Ability to communicate effectively in English. Bilingual provider preferred (Common languages served: Spanish, Somali, Arabic, Amharic)
Knowledge and understanding of socio-economic and health problems of underserved populations are advantageous.
